What is the required form to travel to Spain?
International travelers who want to visit Spain need to complete an online form named “Health Control Form” or, in Spanish, the Formulario de Control Sanitario (FCS). It is to notify the Spanish officials of the health, travel, and the applicant’s information during the visit. When candidates complete the form, the FCS provides a QR code to the applicants.
